FORTITUDE currently accepts United Healthcare, First Choice Health, and Kaiser Permanente. Preferred Provider Organizations (PPOs) and direct pay are also accepted.
Your insurance may still cover the cost of “Out of Network” care, it is recommend potential clients contact their insurance carrier regarding reimbursement of services rendered by “Out of Network” providers.
